Human Services

Phyllis A Holtry is the Community Action Partnership administrator.  Her office is located on 508 Oak Street, Lebanon, across from probation.    Phone 273-0328.

Phyllis and Diane McKinney, the administrative assistant, administer the Human Services Development Fund (HSDF)  and the Homeless Assistance Program (HAP) through the Department of Public Welfare. 

A 12-step program for youth recovering from drugs or alcohol began September 10, 2007.  A peer support group takes place on Monday evenings between 4:30 and 5:30PM.  While Jodi Hoffman, an adult facilitator, is available for guidance, the youth “run” the program.  Juvenile Probation may require some youth to attend.  Further, unless a youth decides to reveal his or her identity, participation may be anonymous, and any youth in the community may attend

The Bridge House, Rental Assistance, and Emergency Shelter fall under HAP.

bulletBridge House is a transitional service that allows clients who are in temporary housing to move to supportive long-term living arrangements (12 to 18 months) while preparing to live independently.
bulletRental Assistance provides rent, mortgage arrearages for home and trailer owners, rental for trailers and trailer lots, security deposits, and utilities to prevent and/or end homelessness or near homelessness by maintaining individuals and families in their own residences.
bulletEmergency Shelter provides shelter assistance in the form of motels for families/individuals who are homeless.
